Herd immunity and security patching: How much patching is enough?

Is it necessary to apply 100 percent of released security patches to 100 percent of vulnerable machines? Is it possible to protect a distributed enterprise network with less than perfect patch application? In my opinion the answers are no and yes, respectively.
